The Post-Lung Transplant Nurse Coordinators report directly to the Medical Director of the Lung Transplant Program with administrative oversight by the Transplant Center Director or their designee. The Nurse Coordinators are responsible for ensuring patients' continuity of care after their discharge phase of transplantation. They serve as the central point of reference for all information about patients participating in the program. The scope of the position includes patient care, education, quality assurance, research, administrative and financial activities, and coordination of transplant care with medical and surgical staff as appropriate.This position is full-time, M-F, 5-8hr shiftsQualifications:Given the high acuity and complexity of these patients:Job Summary
SummaryResponsible for patient care and support within our hospital's outpatient setting through providing compassionate care, assisting in procedures, and collaborating effectively in a dynamic ambulatory care environment.Does this position require Patient Care? YesEssential Functions-Deliver direct patient care and support in our ambulatory care unit, ensuring high nursing standards. -Conduct patient assessments, devise care plans, and assist in outpatient procedures with a focus on patient comfort and safety. -Educate patients and families on treatment plans, medications, and post-procedure care. -Collaborate closely with healthcare professionals to coordinate comprehensive patient care.-Administer medications, perform IV therapy, and maintain accurate patient records in compliance with regulations.-Uphold infection control protocols and maintain a safe environment for patients and staff.Qualifications
EducationAssociate's Degree Nursing required and Bachelor's Degree Nursing preferredCan this role accept experience in lieu of a degree?NoLicenses and CredentialsRegistered Nurse [RN - State License] - Generic- HR Only required Basic Life Support [BLS Certification] - Data Conversion- Various Issuers preferredExperienceClinical nursing experience 0-1 year required and Ambulatory care or outpatient experience 1-2 years preferredKnowledge, Skills and Abilities- Proficient knowledge of nursing principles, practices, and procedures.- Strong communication and interpersonal skills.- Ability to work effectively in a fast-paced and dynamic healthcare environment.- Demonstrated commitment to continuing education and professional development.- Familiarity with electronic medical records (EMRs) and healthcare technologies.
