Pay Range: $70,000-95,000

Responsibilities
- Conduct comprehensive assessments of patients' health and develop individualized care plans.
- Coordinate and manage patient care in collaboration with multidisciplinary teams.
- Monitor patient progress and make necessary adjustments to care plans as required.
- Educate patients and their families on treatment options and care needs.
- Provide direct nursing care to patients in their home settings as needed.
- Document patient condition and care plans accurately in electronic health records.
- Ensure compliance with healthcare regulations and organizational policies.
