Centrally located in Torrance, California, Sunnyside Nursing and Post-Acute Care provides sub-acute nursing care, rehabilitation services, skilled nursing, and long-term care. We understand the importance of creating a comfortable and nurturing atmosphere, whether residents stay for short-term treatment or long-term care.

Job Description

  • Offer coverage on the weekend (Saturday and Sunday) to address psychosocial needs of residents and address any needs related to transportation, grievances, theft and loss, etc.
  • Conduct admission interviews
  • Update residents’ charts/social services records
  • Assisting with scheduling transportation
  • Contact families to pick up personal belonging for resident who return to the hospital
  • Handle arranging ancillary services for the residents
  • Oversee room changes
  • Theft & loss and grievances
  • Conduct Floor rounds
  • Assist with discharge planning
  • Meet with residents/ families to go over plan of care and discharge planning
  • Support clinical team by addressing residents' and families' concerns related to psychosocial issues

Requirements

Ability to work the weekends (Saturday and Sunday)

Prior Social Services Experience

Social Services Designee Certificate

Ability to speak and write English well
