At Crown Hospice we don't just provide care; we provide comfort, advocacy, and a steady hand during life’s most delicate moments.Why This Role MattersAs a vital member of our Interdisciplinary Team, you aren't just managing cases—you’re the bridge between a family’s needs and the resources that make their journey manageable. You’ll be the expert listener, the crisis calmer, and the community liaison who ensures no patient or family walks this path alone.What You’ll Do
- Be the Heart of the Team: Collaborate with clinicians and volunteers to craft personalized care plans that honor the patient’s "wants" as much as their "needs."
- Empower Families: From assessing psychosocial needs to navigating financial assistance and Medicare, you’ll be their champion and guide.
- Connect the Community: Act as a high-visibility ambassador, building bridges with local agencies to expand the reach of hospice philosophy.
- Provide True Presence: Meet patients where they are—literally—by visiting them in their homes and offering counseling that fits their unique situation.
- Qualified: A State Certified Social Worker with a deep understanding of the grief, loss, and the dying process.
- Mobile: Ready to hit the road (valid DL and insurance required) to bring care directly to those who need it.
- Resilient: Calm in a crisis and comfortable working in diverse home environments.
- Expert Communicator: You possess world-class listening skills and the ability to educate staff and the community alike.
