Job Description
JOB SUMMARYThe medical social worker contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement.
DUTIES & RESPONSIBILITIES
Develop standards, policies and procedures for Social Work, Spiritual Care, and Bereavement Programs.
Collaborates with Social Workers and Spiritual Care team members in evaluation and plan of care, and treatment of patient and family.
Meets with social work and spiritual counselor staff to review patient records.
Visits patients at nursing facilities or home to evaluate quality of care and services provided.
Observes for safety hazards in patient’s home/facility environment and takes appropriate action to ensure their safety.
Provides consultation and education to community to increase knowledge related to the hospice and serves as information source and education resource person to community, agency personnel and volunteers as appropriate.
Participates in QAPI Program, Safety Committee and other Committees as needed.
Provides emotional and spiritual support to staff members as needed.
Continue education through academic studies, seminars, and workshops.
Participates in in-service training, staff meetings and case conferences.
Monitors and report status of departmental functioning to management on a regular basis.
Maintains productivity standards of services consistent with hospice standards.
Performs other related duties as assigned.
Supervises Social Workers, Spiritual Counselors, and Bereavement Counselors to ensure state and federal compliance.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
Master’s degree in social services field or a graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in §418.114(b)(3).
Minimum of one year's experience in health care, hospice experience preferred.
Knowledge of Hospice Criteria. Understands hospice philosophy, and issues of death/dying.
Certified, licensed, or registered as appropriate.
Supervisory and administrative experience in hospice or related health-care field.

Possesses and maintains current CPR Certification.
Must be a licensed driver with an automobile that is insured in accordance with the State of Nevada requirements and is in good working order.
