Position Summary

The Social Worker – CCB Case Manager responsibilities include providing Case Management Services and related conditions, assisting individuals in gaining access to needed social, medical, educational and other supports and services.

Assist persons on a waiver to access needed waiver and State plan services, as well as needed medical, social, educational and other services, regardless of the funding source. Assure service planning and delivery are done through a person-centered approach. Assist in monitoring the delivery of services and provider compliance with quality assurance standards.  Complete other functions to support agency operations.

Essential Functions/Responsibilities

The following duties are normal for this classification.  These are not to be construed as exclusive or all-inclusive.  Other duties may be required and assigned.

  • Development and review of a MnCHOICES Support Plan
  • Administrative case management activities

  • Ability to utilize the MMIS and SSIS systems required.
  • Knowledge of and ability to comply with data privacy laws and compliance.
  • Knowledge of the principles and techniques of interviewing and documenting.
  • Knowledge of individual and group behavior.
  • Knowledge of social casework objectives, principles, and methods.
  • Knowledge of social-economic factors.
  • Knowledge of family systems and dynamics.
  • Working ability to work constructively within an agency in the community setting and in effectively utilizing appropriate resources and services
  • Ability to work constructively in the development and coordination of community resources.
  • Ability to manage time productively.
  • Ability to prioritize caseloads.
  • Ability to communicate effectively, both orally and in writing.

Minimum Qualifications

Education and Experience

  • A bachelor's degree from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field or
  • A bachelor's degree from an accredited four-year college or university with a major in any field and one year of experience as a social worker in a public or private social services agency or
  • A public health nurse or
  • A registered nurse

Required Training

  • Training on Person Centered Thinking and Planning. (training provided if not already completed)
  • Mandated Reporter Training. (training provided if not already completed)
  • Annual Data Privacy, Security, Fraud and OSHA Training. (training provided if not already completed)
  • SSIS and MMIS Training. (training provided if not already completed)

Required Certifications/Licenses

  • Valid Minnesota driver’s license with proof of insurance.

Required Certifications

  • Social Worker Licensure preferred

Relationships

  • Individuals needing long term services and support and their families and/or legal guardians.
  • Regular interaction with other County Social Workers, Public Health and Attorney office staff.
  • Medical, psychiatric, psychological, legal, educational, vocational, rehabilitation and other professionals and resources.
  • Department of Human Services staff.

Supervisory Responsibilities

No direction supervision of employees.

Working Conditions

  • Work is mainly performed in an office setting but may also be in clients’ homes, residential facilities, and other county offices.
  • Frequent travel may involve exposure to inclement weather conditions, unsanitary conditions in homes and unsafe environments.
  • May require working with people who may be disoriented, hostile, aggressive, abusive, or threatening.
  • There may be hours worked outside of the normal business hours.

Physical Requirements Of This Position

  • Desk work involves use of a computer, telephone, and other devices.
  • Communicate effectively with County staff, clients, and professional groups.
  • Ability to read screens and documents.
  • Ability to drive in various light and weather conditions.
  • Ability to type and use a computer or mobile device.
  • Office work may include reaching for documents, grasping documents, standing, walking, and repetitive motions.
