Job Description

PURPOSEANDSCOPE:

Supports FMCNA’s mission, vision, core values and customer service philosophy. Adheres to the FMCNA Compliance Program, including following all regulatory and division/company policy requirements. Works closely with VP Social Work Services to develop, plan, and communicate nationwide key clinical initiatives related to social work services. Provides insight, analysis, support, and strategic recommendations for the development, dissemination, and implementation of the various social work services programs and initiatives ensuring alignment with company goals and strategies and compliance with all pertinent regulatory requirements. Communicates and plan the implementation of social work services policies, strategies, and initiatives. Models cross-functional collaboration, continuous quality improvement, and asset-based problem solving. Lead, mentor, and manage team of Strategic Senior Manager Social Work Team to work within their scope and promote professional development. Exercises influential leadership by leveraging key performance indicators to guide Operational partners in maintaining accountability and ensuring the successful and timely execution of projects and programs related to Social Work Services initiatives. Maintains social work licensure and continuing education hours as specified by the Association of Social Board and state licensure regulations where applicable. Regularly monitors registration and licensure status of direct reports. Develops and implements training and education programs.

PRINCIPALDUTIESAND RESPONSIBILITIES:

  • Provide guidance, support,andexpertiseregardingspecificassignedsocialworkservicesprojects underthedirectionoftheVPofSocialWork Services to promote consistent application of strategies, initiatives, and programs.

  • Participates in the development of departmental strategic plans, goals and objectives ensuring alignment with operation and medical office goals. Implements, disseminates, and support operations strategies and objectives to ensure the achievement of the goals and objectives

  • Collaborates with Clinical Quality leadership regarding establishing operational objectives based on business goals to assure alignment. Communicates regularly with other teams such as nutritional services, quality, education, regulatory, clinical services and operations, obtaining feedback and analyzing the recommendations made to contribute to the resolution of related issues and create positive outcomes.

  • Lead Social Work Leadership to support operational teams to drive quality improvement initiatives/activities throughoutFMS to achievestandards identified in CMS Conditions for Coverage and FMS quality requirements. Provide tools to support action planning activities to support patient satisfaction.

  • Technicallyproficientinutilizingappropriatedashboards tomonitorshigh-levelquality trends.Analyzescomplexqualityissues usingin-depthknowledgeoforganizationalobjectives.

  • Responsibilities include but are not limited to evaluating best practices specifically regarding social work services pertaining to clinicalinitiatives,research,quality improvement,anddevelopmentofideastocreatestrategiesfor implementation,process identification and planning, evaluation of the implementation and adoption of clinical initiatives/projects and technology systems.

  • Serves as a subject matter expert, providing clinical guidance, and general direction to operational leadership and facility social work leadership. Promote consistent application of quality social work servicestandards insupport of excellentsocial work care of dialysis patients throughout their assigned business unit.

  • Serve as resource to operational leadership to achieve the social work-related value-based care key performance indicator within assigned territory.

  • Supports the achievement of Quality Incentive Program (QIP) social work-related key performance indicators, such has depression screening and transplant via dashboards to communicate to Operational partners.

  • Acts as a Social Work Services representativeonspecialcross-functional workgroups and company initiatives as assigned by VP of Social Work Services. Decisions are usually complex and have an impact on the business beyond the functional area with serious impact on the overall success or failure in the area of accountability. Erroneous decisions will have a serious impact on the overall success of functional, division or company operations.

  • Collaborates with Social Work Services leadership to develop and implement patient social work educational programs and material for home and in-center patients aimed towards improving patient experience, increasing patient understanding of disease related social work modifications, and improving patient quality outcomes.

  • CollaborateswiththeVPSocialWorkServicesregardingestablishingoperationalobjectivesandworkplansbasedonbusiness goalstoassurealignment.Communicatesregularlywithotherteamssuchasqualityandoperations,obtainingfeedbackand analyzing the recommendations made to contribute to the resolution of related issues and create positive outcomes.

  • Interacts with all levels within the organization to manage functional area. Overall responsibility for hiring, coaching and counseling direct reports, including performance reviews, disciplinary action and terminations.

  • Ensures that budgets and schedules are within company requirements with significant organizational responsibility for the overall control of planning, staffing, budgeting, expense priority management, and recommendation and implementation changes of current methods

  • Provides leadership, guidanceandcoachingfor alldirectreports to maintain anengaged and productive workforce;partnering with Human Resources on employee matters.

EDUCATION ANDCREDENTIALS:

  • MastersDegreeinSocial Work

EXPERIENCE AND SKILLS:

  • 5+years’relatedexperienceoranequivalentcombinationofeducationandexperience.

  • 3+years’experienceas aManager;or3+years’experienceinasenior leadershiprole.

  • Strongmanagementskillswiththeabilitytoleadcohesiveandproductiveteams.

  • Stronginterpersonalskillswiththeabilitytocommunicatewithalllevelsofmanagementthroughdiplomacyandtact.

  • Excellentoralandwrittencommunicationskills
