The Job

The senior manager of care management is an innovator who identifies and overcomes barriers on behalf of members and teams. Oversees care management functions across multiple teams. This role functions in conjunction with the director of care management to set strategic direction for the care team and oversee the work of multiple managers and defines priorities for their daily work. The senior manager of care management ensures the ways of working are implemented and at the forefront of care delivery. Responsible for team compliance with all regulatory requirements, clinical standards and internal policies and procedures. Responsible for reviewing and updating assigned policies and procedures and desktop procedures.

You Will

Directly manages multiple managers of care management and/or care management staff

Oversees case management and care management services for complex or high-cost cases across multiple teams.

Create processes for care managers that reduce time spent on administrative tasks and maximize time spent on member care

Ensures continuity of care with external organizations through building workflows to streamline processes and enhance continuity of care

Develops and administers care management policies and procedures with the goal of balancing quality patient outcomes with cost-effectiveness

Measures effectiveness of care management, quality, or utilization management initiatives by tracking admission/readmission rates, medical resource usage, or other performance metrics

Reviews team activities and operations for compliance with established policies and standards

Identifies barriers, proposes solutions, and initiates communication with cross functional leaders to drive change efforts toward improved member outcomes and efficient processes for teams

Meets weekly with direct reports and keep the Director of Care Management informed about team activities, successes, barriers, and opportunities

Your Qualifications

Bachelor's Degree required.

Registered Nurse (RN) license current & active required.

Graduate or Advanced Degree or equivalent experience preferred.

5+ years of management experience preferred.

Care Management Leadership preferred.

Leadership- Skilled to develops others

Problem Solving- Make critical decisions, often involving high-level risk assessment and the ability to adapt to changing circumstances

Strategic Mindset- Formulates strategy and maps steps to achieve strategic goals
