Job Description
Transfer Center Nurse serves as the primary coordinator for the placement of patients within Winchester Medical Center and system wide in-migration/out-migration of patient movement.
Responsibilities and Duties
Bed Placement
Demonstrates proper assessment of patient needs ensuring proper placement.
Works with physicians to ensure appropriate bed placement for Emergency Department and direct admits.
Assigns beds based on availability, patient care requirements and staffing.
Anticipates discharges and patient bed assignment for the following day (i.e. elective admissions).
Facilitates in-house transfer during peak census time.
Maintains daily log of bed placement activities.
Works with Charge Nurses and Managers to place patients based on productivity in addition to clinical criteria.
Customer Service
Communicates with physician liaison to identify referring physician concerns, needs and opportunity for improvements.
Effectively communicates with WMC departments regarding bed placement.
Patient Transfers
Arranges patient transfers from referring physicians.
Coordinates all patient transfers from referring physician ensuring:
- Assessing appropriateness of request based on patient condition.
- Collect demographics to ensure proper facility placement.
- Facilitates physician to physician contact.
- Facilitates transfer by assigning bed, notify receiving unit, notify bed access, admitting and referring facility of bed assignment and telephone number of receiving unit.
Resource Management
Serves as a liaison with Clinical Administrator to assure we have the capability and capacity to safely place patients.
Education
Bachelors Nursing (BSN) Preferred
Experience
Minimum of 5 years nursing experience with previous management experience preferred.
Certification & Licensures
Current registered nurse licensure, or license eligibility, in the Commonwealth of Virginia
Qualifications
Excellent communication skills.
Ability to work at a fast pace with multiple priorities.
Knowledgeable in the cognitive, physical, emotional and chronological stages of human growth and development.
