Weekend Clinical Supervisor Home Health Part Time

Empath Home Health, a member of Empath Health, is seeking a part-time Weekend Clinical Supervisor (Home Health). The Weekend Clinical Supervisor (Home Health) is accountable for the planning, coordination, and delivery of quality home health care and services; assures compliance with the agency's policies and procedures and applicable federal and state regulations. Provides oversight of all patient care services and personnel.

Location: This is a remote role (work-from-home) supporting Empath Home Health based in FL; out of state applicants may apply.

Schedule: Part-Time, Weekends. Saturday & Sunday (8am-6pm) with on-call duties as assigned; 20 scheduled hours per week plus on-call requirement

Responsibilities:

• Provide oversight of all patient care services and personnel to include: making or assisting the Scheduler with patient and personnel assignments; coordinating patient care; coordinating referrals; assuring that patient needs are continually assessed; assuring the development, implementation and updates of the individualized plan of care are including all physicians involved in the plan of care; and integrating the orders.
• Perform initial and ongoing comprehensive assessments of the patient's needs: Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
• Supervise the nursing clinical staff engaged in direct delivery of home care services in the initial evaluation of patient's needs, the written goal-oriented plan of care, and the implementation of the physician's plan of treatment.
• Coordinate the total plan of care with appropriate resources, conducting periodic reassessments of patient needs and revising the plan of care as needed.
• Ensure the care provided is based on a continual assessment of the patient that addresses physical, psychosocial, environmental and safety factors, physician's orders, relevant medical history, functional status, and availability of caregiver.
• Weekend on-call duties as assigned.

Requirements:

• Degree in Nursing required. Current license as a RN Registered Nurse in the respective state: ARNP or other advance practice nurse
• One (1) year home health care experience (supervisory or higher) required with knowledge of state and federal regulations relative to home health.
• Requires Homecare Homebase (HCHB) EMR experience. Must have thorough knowledge and experience to support staff.
