Job Description

Fraser is looking for Group Home Program Coordinator to support our Community Living team. In this role you will provide supervision and leadership to assure continuous quality improvement in active treatment service development and implantation and comply with all regulations as well as the vision, policies and budgetary parameters of Fraser. You must be 245D qualified for this position.

Qualifications:

You must be 245D qualified in one of the following ways for this position:

  • HS Diploma or Equivalent AND four years of full-time work experience providing direct care services to persons with disabilities
  • BA/BS in Human Services or related field AND 1 year of full-time work experience (or equivalent) providing direct care services to persons with disabilities
  • Associate degree in Human Services or related field AND 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilities

Responsibilities:

  • Assure necessary staff coverage and recommend staffing to meet the needs of the clients served
  • Coordinate and/or complete all household repairs and other upkeep
  • Support other Program Coordinators with on call duties as directed
  • Coordinate the activities of daily living including personal care, hygiene, and grooming
  • Provide direct cares to our clients and supervisory duties
  • Collaborate with medical professionals to coordinate health and medical needs for each client served
  • Maintain accurate financial information for sites
  • This position will serve as a part of an on-call rotation

  • This position is full-time Monday-Friday, standard business hours

  • The starting pay range for this role is $50,000 to $53,000 depending on up qualifications

  • $ 1 000 Hiring Bonus for external new hires
