RN Case Manager (Hospice)

We're hiring an RN Case Manager to lead care for hospice patients—one-on-one, in their homes. No facility chaos. No micromanaging. Just smart, meaningful nursing.
What you'll do:

Handle a small caseload of home-based hospice patients

Do in-home visits, create care plans, and adjust as needed

Teach families what to watch for and how to help

Document visits clearly and on time

Coordinate with doctors, aides, and therapists

Keep everyone aligned—patients, families, your team
What you need:

Active RN license in Idaho
-
At least 1 year of clinical experience (hospice, home health, or med/surg preferred)

Strong clinical decision-making

Able to manage your own schedule

Driver's license + dependable vehicle
