Job Description

Summary

The Advanced Practice Registered Nurse (APRN) and Physician Assistant (PA-C) provide primary and preventive healthcare services to patients across the lifespan. They may deliver care in various locations as needed. Both the PA-C and APRN collaborate closely with CHMG physicians, colleagues and staff to ensure comprehensive, coordinated patient care.

Education

Master's degree in Advanced Practice Nursing or Physician Assistant Studies required.

Certification, Registration & Licensure

  • Current NH licensure as an APRN or Physician Assistant

  • Valid DEA license

Experience

Relevant primary care or site-specific experience required.

Responsibilities

  • Obtain detailed patient health histories, perform physical exams, and develop accurate differential diagnoses

  • Order, interpret, and follow up on diagnostic tests

  • Diagnose and manage acute and chronic medical conditions within the scope of practice

  • Prescribe and manage medications in accordance with practice standards

  • Provide individualized patient education focused on preventive care and health maintenance

  • Meet productivity standards established by the practice or department

  • Accurately document all patient care and interactions in the medical record per departmental protocols
