Position Highlights
The Advanced Heart Failure Coordinator is responsible for coordinating the care of patients with pulmonary hypertension (PH), advanced heart failure, and LVADs in collaboration with a multidisciplinary team. This role includes patient education, clinical coordination, 6-minute walk testing (6MWT), program development, and quality improvement to optimize patient outcomes.
