Job Description
CareArc is seeking a full-time Community Health Manager to join our team.
Duties include:
Responsible for the management and reporting requirements deemed in connection to applicable State grants.
Deliver patient care as appropriate within the scope of nursing license.
Perform other administrative and clerical duties in support of the medical care teams.
Serve as a resource person to other staff in the area of one's own clinical expertise.
Lead in performance improvement activities, maintain ongoing clinical knowledge through internal/external training programs, provide interpretation of knowledge and direction to staff.
Assist in the development and implementation of policies, procedures, and standards which affect patient care.
Supervise staff including scheduling, monitoring time and attendance, performance management and evaluations while also being responsible for task functions.
Participate in the recruitment, interviewing, selection, and orientation of team members.
The ideal candidate is customer service orientated, self-motivated, attentive to detail and can work efficiently both independently and with a team. They also will have a passion for the health and wellbeing of the community.
Education and Experience
- Bachelor's degree preferred, associate's degree required, Field of study: Nursing
- 3+ years experience preferred
