RN - Community Health Manager

CareArc is seeking a full-time Community Health Manager to join our team.

Duties include:

• Responsible for the management and reporting requirements deemed in connection to applicable State grants.

• Deliver patient care as appropriate within the scope of nursing license.

• Perform other administrative and clerical duties in support of the medical care teams.

• Serve as a resource person to other staff in the area of one's own clinical expertise.

• Lead in performance improvement activities, maintain ongoing clinical knowledge through internal/external training programs, provide interpretation of knowledge and direction to staff.

• Assist in the development and implementation of policies, procedures, and standards which affect patient care.

• Supervise staff including scheduling, monitoring time and attendance, performance management and evaluations while also being responsible for task functions.

• Participate in the recruitment, interviewing, selection, and orientation of team members.

The ideal candidate is customer service orientated, self-motivated, attentive to detail and can work efficiently both independently and with a team. They also will have a passion for the health and wellbeing of the community.

Education and Experience

• Bachelor's degree preferred, associate's degree required, Field of study: Nursing
• 3+ years experience preferred
