RN Case Manager, Home Health

A brief overview of Responsibilities:

• Supervise and mentor field staff (RNs, LPNs, and Home Health Aides), including training and onboarding new clinical team members.
• Administer nursing care and clinical skills to homebound patients, establishing and following individualized treatment plans.
• Monitor and report changes in patient symptoms or behavior, ensuring timely communication with the interdisciplinary care team.
• Manage multidisciplinary care-coordinating admissions, transfers, and discharges-to promote continuity and quality of care.
• Educate patients and families on health maintenance, disease prevention, and self-care techniques.
• Document visit notes promptly using the Homecare Homebase electronic charting system.
• Provide emotional support and guidance to patients and their families throughout the care process.

Qualifications:

• Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
• At least one year of home health experience preferred
• Graduate of an accredited nursing school.
• Must have a valid driver's license and a reliable car available during work hours.
• OASIS and HCHB experience PLUS!
