Position Summary:

Responsible for overseeing all aspects of the patient intake process to facilitate timely and efficient hospice admissions. Builds and maintains positive relationships with coworkers, customers, and referral sources while addressing customer inquiries and concerns. Coordinates insurance verification and ensures seamless communication among all parties throughout the admission process.

Hours / Schedule: Full Time; (40) hours / week.

Minimum Qualifications:

  • Diploma or degree in practical or vocational nursing or a related field from an accredited college or university, or the international equivalent (preferred).
  • At least one (1) year of medical-surgical experience.
  • Previous experience in home health or hospice (preferred).
  • Active LPN/LVN licensure in the assigned state.
  • Current CPR certification.
  • Valid driver's license with a reliable, insured automobile that meets state and/or organizational requirements.

Key Responsibilities:

  • Deliver exceptional service to patients, families, referral sources, co-workers, and other stakeholders.
  • Provide support to team members as needed to ensure high-quality service.
  • Exhibit professional and courteous telephone etiquette.
  • Answer and respond to all calls promptly and efficiently.
  • Gather and verify all necessary information for hospice admissions.
  • Communicate promptly regarding any missing or incomplete information.
  • Maintain open and consistent communication with the sales team, clinical team, referral sources, and patients throughout the referral and admission process.
  • Build and sustain positive relationships with current and potential referral sources.
  • Maintain a thorough understanding of contractual agreements and ensure compliance with contract provisions for admissions.
  • Obtain necessary authorization before admitting patients.
  • Accurately document all intake activities for each patient.
  • Enter initial patient information into the electronic medical record system in a timely manner.
  • Follow up on incomplete admissions and referrals that were not taken under care.
  • Assist the community education team in fulfilling outreach and education efforts.
  • Participate in sales calls as needed.
  • Represent the organization at community meetings, civic events, and professional organizations as required.
  • Collaborate with agency leadership to identify challenges in the referral/admission process and suggest improvements.
  • Perform additional intake-related duties as assigned by management to support department goals and initiatives.
