Job Description

ESSENTIAL JOB DUTIES

  • Provide brief integrated behavioral health interventions to patients that may have previous mental health diagnoses, risk factors, or current mental health concerns. Offer crisis intervention as needed, including risk assessment for perinatal mood and anxiety disorders (PMADs). These interventions will primarily take place in Women's Care department but can also be utilized in other departments as needed.
  • Partners with providers, midwives, nurses, doula program, Maternal Infant Health Program, and other clinic staff to ensure integrated, coordinated care. Participates in multidisciplinary team meetings and consultations with Women's care, Primary Care, Pediatric Care, Behavioral Health and community partners, to review patient cases and optimize care delivery. Educates clinic staff and patients on perinatal mental health topics and innovative treatment options to enhance understanding and improve patient outcomes. Acts as a Perinatal Mental Health Liaison for other members of the health care team, providing in-service education and consultation in understanding the social and emotional factors related to health problems.
  • Develop working relationships with leaders in community resource groups to coordinate care across the continuum, to offer patients options, tools and resources. Attends meetings as assigned to promote integration and patient care activities.
  • Provide education to patients and families about perinatal mental health, coping strategies, and available resources. Coordinate referrals to community resources, specialty care, or higher levels of care when necessary. Provides interventions with individuals, families, and groups, including participants in Centering programs at GLBHC as requested.
  • Accurately and promptly documents all patient interactions in the EHR, ensuring timely and precise entry of billing codes for appropriate reimbursement. Communicates promptly with the care team regarding patient needs and necessary follow-ups.
  • Stay updated on emerging research and best practices in perinatal mental health by participating in Post-Partum Support International (PSI) meetings and trainings as scheduled. Also, participate in trainings, workshops, and continuing education to maintain licensure and expertise in providing mental health interventions as well as maintaining PMH certification.
  • Consistently meet productivity standards that are required to maintain program and position sustainability at GLBHC.

MARGINAL JOB DUTIES

  • Performs other duties as assigned by the Behavioral Health-Adult Program Manager.

JOB SPECIFICATIONS

  • Education: Master's degree in social work. Must have or be able to obtain PMH-C certification within 6 months of employment.
  • Licensure: License or limited license in the State of Michigan.
  • Experience: Minimum 3 years' experience in the behavioral health setting required. Experience working with women during the perinatal period highly desired.
  • Skills: Excellent working knowledge of behavioral health medicine and evidence-based treatments for medical and mental health conditions. Must be able to provide quick and accurate clinical assessments of mental and behavioral health conditions with limited patient contact. Maintain accurate, timely, and compliant clinical documentation in the electronic medical record (EMR).
  • Interpersonal Skills: Excellent communication and collaboration skills to work effectively within a multidisciplinary team. Ensure services adhere to ethical, professional, and legal standards, including HIPAA and confidentiality regulations.
  • Physical Effort: Must be able to lift, carry, push, pull, and or twist while holding up to 25 lbs. frequently
  • Hours of Work: Full time; Flexible and varied
  • Travel: Minimal local travel required. Some out of service area travel may be required for conferences and seminars. Reimbursement in accordance with GLBHC travel policy.
