GENERAL DUTIES
Organizational Educational Assessment & Evaluation:
- Assesses patient and healthcare teams' education, information needs and functions that significantly improve patient outcomes and hospital performance i.e., needs related to disease processes, consultant recommendations, administration directives, policy changes, new policy directives and incident reports.
- Evaluates program strategies and success by maintaining and reporting the statistical data related to the screening programs.
- Attends meetings with nursing leadership, clinical teams, educators to determine ongoing needs of patients, nursing, and clinical departments as it relates to the care and treatment of Human Immunodeficiency Virus (HIV), Hepatitis C Virus (HCV) and co-morbidities.
- Develops, facilitates/coordinates follow-up plans for patients and the patients' significant others and counsel patients when informed of results.
Plans, implements, and evaluates the orientation program for all employees:
- Designs, implements, and evaluates instructional and informational interventions.
- Collaborates with patients, staff managers, and supportive departments in developing and implementing patient teaching and care initiatives.
- Plans, implements, and evaluates the screening program for all patients. Collects, aggregates, and analyzes data to measure program success. Creates and submits reports as required to hospital administrators and program representatives.
- Reports all pertinent infectious disease results to LA OPI-1 per guidelines.
- Monitors and verifies project expenses within grant budget and submit invoices when applicable.
Coordinates the development and facilitation of in-service and continuing education programs for all staff:
- Plans, implements, and evaluates continuing patient and nursing education activities.
- Coordinates in-services and training sessions for patients, community and hospital representatives.
- Provides initial and Just-in-Time patient and staff training on an ongoing basis.
Serves as a resource/consultant regarding department-level and house-wide education issues:
- Serves as a consultant and resource for initiatives and disease management projects to internal customers, related to educational and non-educational issues.
- Coordinates skills validation with clinical representatives at LCMC.
- Consults with director, manager, community partners regarding patient education and resources for patient education. Obtains education materials to support and promote current evidence-based practices and teaching.
Serves as a coach to department-level educators and preceptors to develop skills relative to teaching and educational program development:
- Consults with managers, educators and community partners to develop education programs.
- Provides 'Just in Time' coach sessions as needed.
- Mentors clinical staff and clinical support teams.
Coordinates clinical rotations for academic training programs:
- Coordinates affiliations with Frontlines of Communities in United States (FOCUS) and community health care organizations to facilitate patient care and referrals.
- Maintains and communicates schedules of program activities to the leadership.
- Coordinates/facilitates miscellaneous educational activities for patients and staff. Provides input for improving clinical systems and workflow.
EXPERIENCE QUALIFICATIONS
- Required: 3 years of experience in Case Management/Social Work/Staff Development/Education
- Preferred: Previous experience in Patient Navigation/Grant Coordination.
EDUCATION QUALIFICATIONS
- Required: Bachelor's Degree
- Preferred: Master's Degree
LICENSES AND CERTIFICATIONS
Certification Name: Basic Life Support Health Care Provider (CPR, BLS)
Required by end of general hospital/nursing orientation.

KNOWLEDGE, SKILLS, AND ABILITIES
- Excellent communication skills for teaching large and small groups.
- Knowledge of Joint Commission guidelines for hospitals and performance improvement.
- Ability to function as core team member for housewife educational projects.
- Test development, continuing education activities design, implementation, and evaluation.
- Knowledge/experience in various programs such as Corner stone, EPIC, Lawson, and ADP.
- Computer skills (Microsoft).
