Purpose of Position:
The Chaplain for interdisciplinary hospice care team, provides consultation and spiritual support to patients and families facing serious illness, decline in functioning, and emotional and spiritual challenges toward the end of life. Provides Bereavement Coordinator support ensuring excellent patient care services, post-mortem family support and identifying opportunities for improvement in family experience of hospice care.
Job Responsibilities
Chaplain Services
Provide pastoral care for patients and families enrolled in hospice including when moral distress or ethical issues arise, and when complex psychological, spiritual or family dynamics are present
Assist with assimilation of diagnosis, prognosis, physical changes, and functional loss
Provide spiritual care and emotional support to patients and family members to address symptoms of anxiety, depression, and anticipatory grief
Establish clear, patient-centered goals of care
Elucidate what matters most to patients
Provide family education regarding advanced course of illness
Work with family members to resolve conflict and develop consensus around goals of care
Identify 'high risk' flags, such as acute patient or family distress
Offer insights and support during conflicts between patient and family needs
Promote respect for the unique religious and cultural values of patients and families which impact patient care
Provide spiritual care, mentoring and support to staff on issues related to job stress, interpersonal difficulties with patients and families, or moral distress
Consult with Ethics Committee and Quality department as needed
Conduct community outreach to elicit needs
Provide education as requested, internally and in the community
Coordinator for Bereavement Services
- Provides ownership and leadership in the coordination of Bereavement Services for a family after death of a patient.
- Partners closely with Manager of Volunteer Services to determine appropriate interface by volunteers with the bereaved or training needs.
- Owns quality assessment of Bereavement program assuring bereavement risk levels are identified, assessment includes internal guidelines, contacts are occurring.
Job Qualifications
- PhD., M Div., ordination, or equivalent level preferred, with a minimum of 5 years providing hospice or palliative care services
- Experience with ethics consultations preferred
- Must be professional, proactive, collaborative, results-oriented individual with excellent oral and written communication skills, and able to adapt to changing priorities, displaying sound judgment.
- Computer literacy required; experience with Windows, Word, and Excel.
