This position has two 12 hour night shifts (6 PM to 6AM) with rotating weekends, and includes a great shift differential!

Our Licensed Practical Nurses/Registered Nurses are a critical part of our team, who provide care, compassion, and treatment to our residents to ensure their comprehensive needs are met. We are looking for a new LPN or RN to help take the best care of our community, and in turn, have a rewarding career in an environment that values you and will contribute to your continued success. Our Licensed Practical Nurses/ Registered Nurses:

Provide compassionate and effective care and treatments to our residents.

  • Support the Director of Nursing with assessments, admissions, and other duties as assigned.

Work with a team of professionals to ensure the residents comprehensive needs are met.

  • Review routines, calendar of events and resident needs
  • Ensure medications and supplies are ordered, in stock and counted
  • Ensure all charting and record keeping is accurate and complete
  • Promote the Mission, Vision and Values of Transforming Age, our Culture of Excellence, in all areas of responsibility.
  • Other duties as assigned.

What you need to succeed!

Be a graduate of an accredited LPN or RN program

  • Have an active LPN or RN license in the state, in good standing
  • Experience in Long-term Care
  • Have flexibility, able to work the occasional weekend/holiday as needed.
  • Provide leadership and supervision to nursing team members
  • Have knowledge of DHS Regulations
  • Possess superb interpersonal and communication skills and the ability to work well and collaborate with team members and external constituents

Physical Requirements:

  • On occasion, may need to push/pull/support up to 50lbs.
