What You'll Do:
- Deliver skilled nursing care in the home
- Follow and contribute to the care plan
- Communicate changes in condition with the care team
- Educate and support families
- Document care using our electronic charting system
Requirements
- Current RN or LPN license
- Strong communication and organizational skills
- A compassionate, professional approach to care
