Position Highlights
Position Summary:
The RN - Discharge Navigator works collaboratively with multidisciplinary teams in both the inpatient and outpatient settings of the Avera Heart Hospital to facilitate and ensure adequate post-discharge follow up and testing for patients primarily with North Central Heart. This may include the facilitation and organization of post-discharge appointments with primary care and specialty clinics as well, and serving as a resource for patients, families and nursing related to discharge processes. The RN-Discharge Navigator is a liaison among providers, nursing staff, patients and families.
Work Schedule:
Monday through Friday 8:00 am-5 pm, variable day shift, with occasional early morning/evening/weekend involvement.
Work Environment:
Work is typically performed between the hospital and clinic environments. High contact with other staff, physicians, patients, families, and representatives from outside agencies. The noise level in the work environment is usually moderate.
Education/Experience:
Bachelor's degree in nursing preferred.
Minimum of 5 years cardiovascular nursing experience required; Clinical experience with acute and ambulatory patient flow and knowledge of scheduling post-discharge follow up preferred.
Licensure/Certification:
Registered Nurse with current state licensure or licensure from a state within the Nursing Compact. MN licensure required within 6 months of start date. Current BLS and ACLS certification
