Job Summary

Job Duties include the following, other duties may be assigned:

  • Performs a psychosocial and bereavement assessment and assists in the development and implementation of goal-directed plan of care.
  • Conducts ongoing reassessments of patient/family needs and counsels as required.
  • Provides short-term crisis intervention and individual or family counseling when indicated.
  • Provides social work therapeutic intervention through counseling and education to patient/family to maximize strengths and coping capacity.
  • Promotes patient self-determination and self-advocacy.
  • Provides financial counseling and assists in negotiating fiscal systems, particularly government entitlements.
  • Identifies and coordinates community resources as well as defines and documents unmet needs and gaps in service.
  • Collaborates with the IDG through consultation, advocacy on behalf of staff, and facilitation of group process among staff members.
  • Informs IDG regarding psychosocial, spiritual, physical, and financial conditions affecting the patient/family.
  • Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.
  • Participates in programs and educational activities such as bereavement support groups, volunteer training, staff education, and other programs.
  • Participates in orientation and education of new staff, students, volunteers, and others as appropriate.
  • Participates in case conferences as part of the interdisciplinary team as indicated.
  • As assigned, functions as a bereavement counselor for a selected group of patients.
  • Documents according to agency policy and procedure on a timely basis.
  • Conforms to established Agency expectations of productivity.

Education and Experience

  • Bachelor's or higher in Social Worker from a CSWE accredited institution required.
  • Training and/or equivalent work experience in the area of community service required.
  • One (1) year social work experience in a health care setting required.
  • Geriatric or home care experience desired.
  • State certification(s) as requested.

Certificates/Licenses

  • Licensed Clinical Social Worker or Licensed Social Worker (LCSW or LSW)
  • Valid Driver's License
