Job Description
Your job is more than a job.
You're the light at the end of the tunnel as patients begin their rehabilitation journey at LCMC Health. As a Community Liaison Nurse, you're vital to the process of healing, hope and home as you connect patients, physicians and healthcare facilities to our nationally accredited rehabilitation services. Understanding that continuity of care is a major step in recovery, you assist in determining the appropriate patient program, conduct pre-admissions assessments on prospective patients, review their medical records, discuss patient needs with the interdisciplinary team and medical staff, educate the patient and family on our programs, and facilitate the most appropriate level of care.
Excellent communication skills in dealing with patients and family members at very difficult times.
As the flagbearer for rehabilitation services, you champion our expertise and programs by developing relationships with referral sources. You establish yourself as trusted resource with clinical knowledge of inpatient rehabilitation admission criteria and a reputation for "dotting the I's and crossing the t's." Your Everyday
Evaluate potential rehab candidates outside Touro Infirmary within 24 hours of referral, and function as a liaison between TRC staff and referral sources.
Input assessment into UDS ProDoc to ensure timely submission to physician for review and insurance authorization by the pre-admission case managers.
Facilitate expeditious transfer of appropriate candidates to rehabilitation inpatient programs. Ensure a reasonable discharge plan is identified prior to admission.
Inform referral source, patient, and family of next steps in the admissions process.
Develop, build, and maintain professional relationships with current or potential referral sources.
Provide community education by speaking to professional community about IRF criteria and types of diagnoses served.
Assist with marketing efforts and customer satisfaction to ensure continued referral base and expand potential new clients.
Educate referred clients on rehabilitation services to increase awareness and understanding of specialized stroke, brain injuries, and trauma recovery programs.
The Must-Haves
Minimum:
Current nursing license to practice in Louisiana and as defined by the Louisiana State Board of Nursing.
Current American Heart Association BLS certification.
3 years of experience Acute care or rehabilitation nursing
