Proactively monitor the external scientific landscape to identify emerging research and advantaged claim opportunities against priority health areas. Ensure consumer-facing marketing materials and product label claims are aligned with the supporting science. Interface with suppliers, contract research organizations and researchers to identify and develop science-based next generation ingredients against prioritized benefit areas. Actively participate in the new product innovation process from discovery and initial feasibility assessment through concept development and commercialization. Assess new ingredients and technologies to inform innovation pipeline and opportunities for value creation. Nutrition Assistants provide food service support in multiple ways to include: procuring and delivering meals from the central kitchen to sites, preparing necessary foods on-site for mealtime, supporting mealtime in the classroom as needed, and maintaining a safe and sanitary kitchen space at the designated site. Responsibilities : Provide support to volunteers working in the Corner Market to assist shoppers Restock the Corner Market as needed, maintaining a neat and organized inventory Update stock in the inventory management system Assist online order shoppers at their scheduled pickup time Assist in the Food Pantry warehouse: assembling packages of groceries and frozen meals for distribution, sorting and storing food donations and fresh rescue donations Assist shifts of kitchen volunteers to portion and package frozen meals, and other side work in preparation for the following morning Conduct fresh rescue food pickups and assist with gleaning, sorting, and organizing as needed Operate manual and electric pallet jacks as well as use a forklift Provide safe and nutritious meal delivery to older adults Drive company vehicles and adhere to a strict delivery or pickup schedule Maintain the temperature of all meals and complying with food safety guidance and monitoring Complete pre trip safety inspections Complete food temperature logs on route Report and client issues or concerns Skills/Experience/Abilities That Are a Must-Have: Must have a valid CA driver's license and willingness to drive company vehicles ranging in size Responsible, well organized Comfort in using a computerized client management system Ability to work independently and collaboratively as part of a team Excellent verbal, written, interpersonal, and organizational skills Able to work with a wide range of individuals from a diverse socio-economic background and origins Clean DMV record Ability to push/pull/lift 50 lbs Ability to sit, stand, walk, climb stairs Ability to provide friendly, client-centered customer service and public interactions Ability to drive, inspect and maintain vehicles including catering (hot/cold) trucks Skills/Abilities We’d Like You to Have : Bilingual preferred This position is subject to random drug and alcohol testing in accordance with DOT regulations Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential duties and responsibilities of this job. About the Role The Seen Health Nutrition Services role works alongside the Registered Dietitian (RD) to help support nutrition-related services for older adults. This full-time Nutrition Services expert is accountable for assisting in the provision of safe, therapeutic nutrition services at Seen Health. This role receives vendor-prepared food, prepares it for participant meal service, verifies the temperatures of cooling and warming equipment are recorded as directed, and ensures that the kitchen area remains clean and sanitary at all times. Responsibilities Assist the Registered Dietitian to ensure all meals meet the therapeutic and cultural dietary needs of Seen Health participants. Updates information on dietary card and diet lists when information changes or as needed. Prepares and serves morning and afternoon snacks according to snack menu, portion control, diet orders, and time constraints. Receives, handles, and maintains vendor-prepared food in accordance with acceptable food storage standards. Measure, monitor and record food temperatures upon delivery, ensuring food remains safe and within the ranges as outlined in Seen Health policies and procedures. Prepares and serves morning and noon meal at a proper temperature with references to; diet, supplements, adaptive equipment needs and portion control. Label and date all perishable items in Seen Health refrigerators and freezers, including any pre- packaged home-delivered meals, ensuring freshness and safety. Maintains the inventory and restocks supplies on a regular basis. Monitors related expiration dates. Assists the Registered Dietitian with ordering, tracking, and delivering meals, snacks, food supplies, supplements, kitchen inventories and cleaning supplies, and place orders for replacements as needed to ensure consistent kitchen operations. Communicates with the Registered Dietitian regarding nutritional issues. Uphold cleanliness and sanitation protocols to promote a safe and inviting kitchen environment for both staff and participants.   • Conduct personalized nutrition therapy consultations via telehealth
  • Administer evidence-based clinical protocols and deliver highly individualized quality nutrition support
  • Monitor patient progress and make adjustments, when needed, to help patients reach their goals and improve clinical outcomes
  • Achieve high patient retention rates by following evidence-based clinical program guidelines
  • Collaborate with other key members of the care team
  • Leverage meal planning tooling that assists with patient compliance and execution of care plans
  • Participate as needed in clinical rounds with partner practices and their care teams
  • Participate in quarterly RD group meetings and monthly 1:1s with CNM
  • Support patients in making lifestyle changes
  • Develop a relationship with a patient and/or their caregiver quickly to build and maintain trust 
  • Demonstrate empathy to each patient or caregiver
  • Document clinical information in an Electronic Medical Record
  • Ability to work in a fast-paced, constantly evolving environment with a positive attitude
  • Ability to transition between patients efficiently and follow operational procedures in line with clinical programming

Requirements

  • State license in Illinois
  • Bachelor's Degree in Nutrition/Dietetics
  • Completion of Accredited Dietetic Internship
  • Credentialed by the Commission on Dietetic Registration (CDR)
  • Minimum 1-2 years of post-internship nutrition experience
  • Experience counseling patients virtually or in an outpatient setting
  • Expertise in treating patients with GI conditions like IBD, IBS, fatty liver, celiac, etc.
  • Comfort in educating patients with complex chronic conditions including diabetes and kidney disease
