Employment Type: Part time Shift: Description: Posting GENERAL SUMMARY: Responsible for the coordination and oversight of outpatient diabetes and nutritional education services. Works collaboratively with the Medical Director(s), physicians, inpatient diabetes and nutrition resources, allied health providers, physician organizations and the community. Develops, implements and evaluates patient and professional education. In collaboration with SJMHS leadership, is responsible for establishing a plan for continuous improvement in program quality, program volume and community outreach. Provides supervision and coordination to the interdisciplinary team that includes registered dietitians, registered nurses, program instructors, student interns, clerical support and volunteers. The program coordinator functions as a program liaison across the continuum and is responsible for program outcome measurement as well as certification of the diabetes education program. ESSENTIAL DUTIES and RESPONSIBILITES: Identifies and recommends program goals and objectives for the Saint Joseph Mercy Center for Diabetes and nutrition services and facilitates staff participation to reach the desired outcomes. Coordinates daily operations by overseeing scheduling of clinic staff, activities of clerical staff and volunteers, scheduling, registration, billing of patients, and documentation and implementation of services. Serves as the chairperson for the Diabetes Advisory Committee. Seeks out opportunities to expand and enhance diabetes and nutrition services relative to the demands of the patients, physicians and the community. Develops, implements, monitors and evaluates standards, policies and procedures related to the delivery of diabetes education and nutrition services in alignment with JCAHO requirements. Takes the lead for maintaining updated diabetes curriculum, educational materials and programming to provide current content based on changes in research and the health care industry. Prepares and organizes written reports to credentialing agencies and for community benefit accounting as applicable in a timely manner. Conducts data analysis and maintains statistics as applicable for certification and program review. Manages and updates diabetes and nutrition databases as needed to collect outcome data required for program certification(s). EDUCATION REQUIREMENTS: Education, Experience, Licensure. Requires a Bachelor's Degree in Nutrition or Nursing. Master's degree preferred in Nutrition, Nursing, Adult Education or Public Health. Registration in field required. Certification in Diabetes Education through the National Certification Board for Diabetes Educators required. Experience in the NICU highly preferred Five to seven years progressively more responsible related work experience in diabetes and/or nutrition services. Evidence of skills in leadership, creativity, teaching, education, communication and group dynamics. A strong working knowledge of data base management and experience with current database system. REQUIRED SKILLS and ABILITIES: Strong interpersonal skills to communicate effectively with a diverse patient population, peers, staff and physicians. Analytical ability to evaluate clinical and operational data for program monitoring and evaluation. Leadership skills and ability to coordinate a clinical team and fosters a positive and engaged work team. Writing skills sufficient to prepare written material in a clear, concise and organized manner. Good organizational skills with ability to prioritize multitude of responsibilities and duties. Ability to type on a computer keyboard and good working knowledge of computer software programs. Advanced teaching and presentation skills. GENERAL SUMMARY:

Responsible for the coordination and oversight of outpatient diabetes and nutritional education services. Works collaboratively with the Medical Director(s), physicians, inpatient diabetes and nutrition resources, allied health providers, physician organizations and the community. Develops, implements and evaluates patient and professional education. In collaboration with SJMHS leadership, is responsible for establishing a plan for continuous improvement in program quality, program volume and community outreach. Provides supervision and coordination to the interdisciplinary team that includes registered dietitians, registered nurses, program instructors, student interns, clerical support and volunteers. The program coordinator functions as a program liaison across the continuum and is responsible for program outcome measurement as well as certification of the diabetes education program.

ESSENTIAL DUTIES and RESPONSIBILITES:

Identifies and recommends program goals and objectives for the Saint Joseph Mercy Center for Diabetes and nutrition services and facilitates staff participation to reach the desired outcomes.

Coordinates daily operations by overseeing scheduling of clinic staff, activities of clerical staff and volunteers, scheduling, registration, billing of patients, and documentation and implementation of services.

Serves as the chairperson for the Diabetes Advisory Committee.

Seeks out opportunities to expand and enhance diabetes and nutrition services relative to the demands of the patients, physicians and the community.

Develops, implements, monitors and evaluates standards, policies and procedures related to the delivery of diabetes education and nutrition services in alignment with JCAHO requirements.

Takes the lead for maintaining updated diabetes curriculum, educational materials and programming to provide current content based on changes in research and the health care industry.

Prepares and organizes written reports to credentialing agencies and for community benefit accounting as applicable in a timely manner.

Conducts data analysis and maintains statistics as applicable for certification and program review.

Manages and updates diabetes and nutrition databases as needed to collect outcome data required for program certification(s).

EDUCATION REQUIREMENTS:

Education, Experience, Licensure.

Requires a Bachelor's Degree in Nutrition or Nursing. Master's degree preferred in Nutrition, Nursing, Adult Education or Public Health. Registration in field required. Certification in Diabetes Education through the National Certification Board for Diabetes Educators required.

Experience in the NICU highly preferred

Five to seven years progressively more responsible related work experience in diabetes and/or nutrition services.

Evidence of skills in leadership, creativity, teaching, education, communication and group dynamics.

A strong working knowledge of data base management and experience with current database system.
