Position Requirements:
- Licensed Social Worker in California (LCSW or MSW preferred).
- CPR Certification.
- Recent physical exam within the past year.
- Clear TB Test.
- Strong communication and interpersonal skills.
- Ability to work independently and manage a flexible caseload.
Role Responsibilities:
- Evaluate and support patients in their homes, focusing on emotional well-being, family dynamics, discharge planning, and resource coordination.
- Provide support to patients and their families in navigating healthcare systems and connecting with community resources.
- Document progress with easy-to-use laptop software.
- Manage a caseload of 1 to 7 patients per week, ensuring each patient receives compassionate and personalized care.
