Responsibilities

• Provide hands-on personal care to clients, such as assisting with bathing, dressing, grooming, and mobility, ensuring their comfort and dignity at all times.
• Help clients with daily living activities including meal preparation, feeding, light housekeeping, and medication reminders to promote their independence and well-being.
• Monitor clients’ health and report any changes or concerns promptly to nursing staff or family members, ensuring safety and proper care coordination.
• Offer companionship and emotional support to foster a positive and encouraging environment, helping clients stay engaged and mentally stimulated.
• Maintain accurate and detailed records of care provided and client progress to ensure continuity and quality of care.
• Adhere to all company policies, safety procedures, and infection control standards to provide a safe and professional service.

Requirements

• Current certification as a Home Health Aide is required.
• Must have worked with a home care agency within the last 2 years.
• A compassionate and patient nature with a genuine desire to help others maintain their independence and quality of life.
• Strong communication skills and the ability to follow detailed care plans and instructions accurately.
• Reliable transportation to commute between Southold and Riverhead locations, ensuring timely attendance and flexibility.
• Ability to pass background checks and possess necessary immunizations or health screenings in keeping with client and company standards.
