Job Description
Background/Education Required:
High school diploma or GED, or experience sufficient to equal same. One year’s experience with individual with disabilities preferred. Ability to read, write and speak English.
Physical Requirements for Position:
- Physical lifting, pushing, pulling, stooping, bending associated with working with adults with disabilities.
- Ability to navigate ad provide support in the community the individual lives in.
Special Requirements for Position:
- Valid driver’s license that meets agency’s standards (ongoing throughout employment)
- Completion of all mandatory training required by NYS and Agency
- Completion of criminal background check, abuse and neglect background check and child abuse registry background check and subsequent clearance by OPWDD.
- Position works flexible hours, including evenings, weekends and holidays.
- Access to a computer to be able to complete paperwork including timesheets, expense sheets, mileage sheets, receive important notifications, complete trainings and other agency related forms.
Duties & Essential Elements (not inclusive)
- Follows individual’s plan of supports as outline in the Life Plan/ Staff Action Plan. Responsible for actively working with the individual to attain valued outcomes including in home, in the community and at work supports.
- Exhibits sensitivity to and respects the rights of the individual including their emotional and physical needs. Communicates needs or desires of the individual in both verbal and written form via protocols established by individual and their family.
- Is active member of the Circle of Support
- Maintains necessary documentation as required by the individual’s plan, New York State and Medicaid guidelines. Submits required documentation including timesheets, billing sheets and expense reports in a timely manner adhering to deadlines.
- Provides teaching, supervision, assistance and monitoring in areas related to the acquisition and retention of daily living skills if outlined in the individual’s plan of service. Examples include but aren’t limited to:
- Planning and preparation of nutritionally balanced meals
- Care of the home and personal belongings of the individual
- Documentation of money management
- Use of community resources
- Personal care aid duties including but not limited to bathing, toileting, dressing, etc.
- Support needed to be successful at work or volunteer opportunity
- Facilitates community integration and inclusion by following through with activities of interest identified by the individual.
- Transports individual served in personal or family owned vehicles as needed, requested, or scheduled. Adheres to all local, state and federal transportation guidelines to ensure safety of the individual being transported.
- If provides supports as a job coach will work with individual while at job or as a support staff for a volunteer opportunity to provide the necessary support to help ensure success of the individual while at work or at their volunteer opportunity. Will work with individual’s employer/ supervisor to help ensure individual is learning the job, able to be independent in their responsibilities and follows employer’s rules and policies.
- If provides Job Development the ability to meet and evaluate readiness for employment. Seek out and help attain employment. Provide other necessary support as deemed important by individual or circle of support.
- Demonstrates flexibility in order to meet the needs of the individual.
- Understands and complies with agency and OPWDD regulations and policies including practices the Code of Conduct/Ethics and reports all suspected, observed, discovered abuse and neglect.
- Maintains a safe, orderly environment
- Performs other duties as requested or required
