• Collaborate with patients, families, physicians, and hospital staff to develop and implement individualized care and discharge plans.
• Coordinate and streamline patient care progression across various healthcare settings, ensuring safety, efficiency, and satisfaction.
• Assess social, emotional, and financial factors affecting patient health, and recommend appropriate resources and placements, including rehab, nursing facilities, homecare, and community services.
• Act as a patient advocate, negotiating with payors and service providers to secure necessary resources.
• Monitor length of stay, identify barriers to discharge, and resolve delays or obstacles proactively.
• Educate patients and families about care options, support cultural diversity, and ensure understanding of treatment and post-discharge care.
• Ensure compliance with all regulatory requirements and organizational standards, actively participating in performance improvement initiatives.

Required Skills:

• Current RN or LMSW license in NM.
• Minimum of 2 years clinical experience, including at least one year in acute care.
• Strong communication, critical thinking, and conflict resolution skills.
• Ability to multitask and adapt in a fast-paced environment.
• Knowledge of discharge planning, utilization management, case management, and community resources.
• Proven ability to work independently, exercise sound judgment, and collaborate effectively.

Nice to Have Skills:

• Certification in Case Management.
• BLS certification (AHA).
• Experience in multidisciplinary coordination and patient advocacy.

Preferred Education and Experience:

• BSN preferred; MSW or higher social work degree strongly preferred.
• Successful minimum of 5 years' experience in a similar care management role, or equivalent success in CHRISTUS Care Manager I if transitioning from another position.

Other Requirements:

• Travel as needed within healthcare facilities.
• Willingness to work five 8-hour days per week.
• Engagement in continuous professional development and compliance with licensure standards.
