Position Responsibilities

Responsibilities include but are not limited to:

✔️ Providing skilled nursing care and prescribed treatments✔️ Assessing patient conditions and implementing nursing care plans✔️ Monitoring and evaluating patient progress✔️ Coordinating care with physicians, therapists, nutritionists, and social workers✔️ Supervising LPNs, CNAs, HHAs, and subordinate staff as needed✔️ Identifying physical and emotional health concerns through assessment and observation✔️ Maintaining accurate nursing documentation and records✔️ Supporting patients and families with education and guidance

Qualifications

✅ Active New York State Registered Nurse (RN) License REQUIRED✅ Previous nursing home or long-term care experience preferred✅ Strong clinical judgment and communication skills✅ Ability to work independently and collaboratively within a healthcare team✅ Compassionate and patient-focused approach to care
