Position Summary:

Unified Care Hospice is seeking a dedicated and compassionate Medical Social Worker to join our dynamic team. The Medical Social Worker plays a crucial role in our patient care team, providing support to patients and their families to ensure they receive the best possible care during their hospice journey. The successful candidate will have a strong background in social work, excellent communication skills, and a commitment to the highest standards of care.

Key Responsibilities:

• Conduct comprehensive psychosocial assessments for patients and their families to identify their unique needs and challenges.

• Develop and implement individualized care plans, in coordination with the interdisciplinary hospice team, to address the emotional, social, and practical aspects of care.

• Provide emotional support, counseling, and education to patients and families to help them cope with end-of-life issues.

• Advocate for patients and assist in connecting them with community resources, financial assistance, and other support services.

• Collaborate with the interdisciplinary team, including physicians, nurses, chaplains, and other professionals, to provide holistic care to patients.

• Maintain accurate and up-to-date documentation of patient assessments, care plans, and interventions.

• Participate in regular team meetings to discuss patient care and provide input on care plans.

Qualifications:

• Master's degree in Social Work (MSW) from an accredited institution.

• State licensure as a Social Worker.

• Previous experience in a healthcare or hospice setting is preferred.

• Strong communication and interpersonal skills.

• Compassionate, empathetic, and dedicated to delivering the highest quality of care.

• Ability to work effectively as part of an interdisciplinary team.

Pay: Salary based on experience

• Rotating On Call
