Job Description

Position Overview:Unified Care Hospice is seeking a compassionate and dedicated Registered Nurse Case Manager to join our mission-driven team. As an RN Case Manager, you will play a pivotal role in ensuring comfort, dignity, and quality of life for our patients and their families. Key Responsibilities:

  • Conduct comprehensive physical, emotional, and psychosocial assessments, developing individualized care plans in collaboration with our interdisciplinary team.

  • Provide expert pain and symptom management while supporting the unique needs and goals of each patient.

  • Work closely with physicians, social workers, chaplains, and caregivers to ensure seamless, coordinated care.

  • Offer compassionate education and emotional support to patients and families, empowering them to make informed decisions about end-of-life care.

  • Maintain timely and accurate documentation in electronic medical records (EMR) in compliance with regulatory requirements.

  • Participate in interdisciplinary team meetings, contributing insights that enhance patient-centered care.

  • Serve as a resource for patients, families, and healthcare providers on pain management, symptom control, and hospice philosophy.

  • Engage in ongoing education and professional development to uphold the highest standards of hospice care.

  • Support Unified Care Hospice’s growth by referring qualified professionals who align with our values and mission.

Qualifications:

  • Current Registered Nurse (RN) license in the state of Nevada (required).

  • Hospice or home health experience preferred, but not required—we provide training and mentorship.

  • Strong communication and interpersonal skills, with a deep sense of empathy and connection to patients and families.

  • Proficiency in electronic medical records (EMR) systems and documentation.

  • Ability to collaborate effectively within an interdisciplinary team.

  • Reliable transportation for patient visits (mileage reimbursement provided).

    • Comfortable with travel and a flexible schedule.

  • COVID-19 vaccination required.

Schedule & Availability:

  • Day, Evening, Night, or Overnight shifts available.

  • Monday–Friday with rotating on-call responsibilities.

  • Additional hours may be available on holidays or as needed.
