Adult Case Manager

UCP of Maine is looking for compassionate, responsible, organized, self-motivated professional who can work independently and as a member for a team The ACM is responsible for coordinating support services, facilitating team meetings, developing Person Centered Plans, advocating for consumers, encouraging self-advocacy and empowerment, and assisting individuals to be active participants in the community and expand upon their life experiences. The individual must have effective oral and written communication skills, be able to manage challenging situations, operate as part of a larger team, and understand the importance of their role and the impact on the lives of those they are serving.
