Job Description
JOB SUMMARY:
The APP provides primary health care and performs selective medical services under the direction of supervising physicians and within the scope of the approved utilization plan. Additionally, the APP will perform community and/or school health promotion/disease prevention activities as directed.
Schedule:
- 0.9FTE (36 hours)
- Variable Days
- 12hr Shifts
- Rotating Weekends Required
ESSENTIAL FUNCTIONS, DUTIES AND RESPONSIBILITIES:
- Obtains complete medical history and physical data on patients.
- Reviews patient records to determine health status.
- Make decisions regarding data gathering and appropriate management and treatment of patients being seen for the initial evaluation of a problem or the follow-up evaluation of a previously diagnosed and stabilized condition.
- Prepares written prescription orders for drugs and controlled substances.
- Interviews, advises and instructs patients and family regarding medications, treatment, health and illness prevention.
- Recommends community resources to meet patient and family needs.
- Triages patient telephone calls and provides consultation.
- Performs independent evaluations and treatment procedures in emergency situations.
- Attends regular meetings and participates in committees as requested.
- Participates in professional development activities and maintains professional affiliations.
- Arrange for or refer patients to needed services not provided at the clinic.
- Identify normal and abnormal findings in history, physical examination and commonly performed laboratory studies
ADDITIONAL RESPONSIBILITIES:
- Reports directly to physician when required.
QUALIFICATIONS (Required):
- Completion of an approved APP program.
- Minimum of 2 years (4 preferred) experience independently practicing; urgent care or outpatient experience highly preferable.
- Must be able to work independently, as there will be shifts when an APP in the only clinician onsite.
- Possesses appropriate state license to practice as an APP.
- Current DEA.
- Certification from National Certification Organization.
- Must have a mature personality, sense of responsibility and integrity.
- Must be flexible and willing to learn new skills.
- Must be level headed and calm in emergencies and under stress.
- Skill in establishing and maintaining effective working relationships with patients, medical staff and the public.
- Able to communicate clearly, both verbal and written.
- Strong personal initiative required.
- In all matters this person shall observe the confidentiality of patient records and information.
