Job Description
The Advanced Practice Provider (APP) provides comprehensive nephrology and kidney transplant care to individuals, families and groups in the inpatient setting. Responsibilities include the diagnosis and management of acute and chronic kidney disease, management of kidney transplant recipients, and coordination of care for complex renal conditions. The APP will emphasize health promotion, disease preventions, dialysis management, and transplant aftercare. This role provides the opportunity to practice autonomously and collaboratively within an interdisciplinary team of physicians, surgeons, APPs, nurses, and other specialist. All TGH team members are responsible for performing job duties in alignment with the mission, vision, and values of TGH.
Responsibilities
Evaluate, diagnose, and manage acute and chronic kidney disease, electrolyte disturbances, and glomerular disease in the inpatient setting
Provide post-transplant care for kidney transplant patients, including immunosuppressive medication management, rejection surveillance and management, infection monitoring and treatment, and coordination of care with nephrologist and transplant surgeons
Manage dialysis-related care, including hemodialysis, peritoneal dialysis and continuous renal replacement therapy in ICU settings
Perform comprehensive assessments, order and interpret diagnostic and laboratory testing, and develop evidence-based management plans
Provide care for nephrology patients int he ICU, including dialysis
Collaborate with the interdisciplinary team to optimize outcomes and ensure continuity of care
Participate in nephrology consult services including transplant recipients, complex renal cases, and critically ill patients
Works autonomously as well as in collaboration with a variety of individuals to diagnose and manage clients' health care problems
Education
- Master's degree Nurse Practitioner or Physician Assistant
License - Issued by Florida or Compact State
- Advanced Practice Registered Nurse or Physician Assistant
Certification
- Acute Care certification, Adult Gerontology Acute Care certification or Nationally Accredited Physician Assistant Board certification
Work Experience
Experience as Advanced Practice Registered Nurse or Physician Assistant preferred
