JOB SUMMARY - Outpatient Palliative Care
The Advanced Practice Provider (APP) delivers comprehensive outpatient palliative care services to individuals and families with serious, chronic, or life‑limiting illnesses. Care is focused on symptom management, quality of life, goals‑of‑care discussions, and psychosocial support across the disease trajectory. The APP practices both autonomously and collaboratively within their established scope, partnering closely with physicians and interdisciplinary team members. All team members are responsible for performing job duties in accordance with the mission, vision, and values of Tampa General Hospital.
RESPONSIBILITIES - Outpatient Palliative Care
- Provides comprehensive outpatient palliative care evaluations and follow‑up visits
- Manages complex symptoms including pain, dyspnea, nausea, fatigue, anxiety, and depression
- Conducts goals‑of‑care discussions, advance care planning, and code status conversations
- Collaborates with patients, families, referring providers, and interdisciplinary team members to develop and implement individualized care plans
- Coordinates care across settings, including specialty clinics, primary care, home health, and hospice services
- Orders, interprets, and evaluates diagnostic and laboratory tests as clinically indicated and in accordance with departmental protocols
- Provides patient and family education regarding disease trajectory, symptom management, and available support services
- Documents clinical encounters accurately and timely in the electronic medical record
- Serves as a clinical resource for palliative care principles and best practices within the outpatient setting
- Participates in quality improvement initiatives and promotes patient‑centered, evidence‑based care
TECHNICAL KNOWLEDGE, SKILLS & ABILITIES
Demonstrates current knowledge and clinical expertise in palliative care utilizing evidence‑based practice and established protocols
Strong assessment and clinical decision‑making skills in managing complex, chronic conditions
Excellent communication skills, including proficiency in sensitive and difficult conversations
Ability to work autonomously while collaborating effectively with an interdisciplinary care team
Proficiency in Microsoft Office applications and electronic medical record (EMR) systems, preferably Epic
Ability to multitask and prioritize in a dynamic outpatient clinical environment
Demonstrates professionalism, empathy, cultural sensitivity, and outstanding customer service skills
Current licensure as an Advanced Practice Registered Nurse or Physician Assistant in the State of Florida
National certification required for APRNs and PAs
Experience in palliative care, hospice, oncology, geriatrics, or complex chronic disease management strongly preferred
Strong interest in serious illness care and interdisciplinary collaboration
New graduates with strong interest in palliative care may be considered, including participation in structured transition‑to‑practice or fellowship opportunities
