A Brief Overview

Responsible for the daily management of community-based home health and hospice services, ensuring the delivery of high quality, safe, and efficient patient care. The Agency Manager serves as the Agency Administrator and is appointed by the governing body.

What you will do

  • Demonstrates ability to communicate, plan, and be responsible for all day-to-day operations of the agency, including the execution of strategic goals.
  • Ensures that the Agency employs qualified personnel, including assuring the development of personnel qualifications and policies.
  • Provides input for budget preparation; monitors expenses; develops plan of action to correct variances, and achieves agency growth targets.
  • Monitors patient care and agency processes to assure safe, customer-focused approach. Implements and leads change initiatives to impact operational and quality outcomes.
  • Demonstrates positive collaborative relationships with individuals, groups, health facilities and referral sources, becoming the face of Avera.
  • May serve as a liaison in meeting with patients and families in the hospital, long-term care facilities, and clinics to assess and evaluate patient for home care appropriateness.
  • Monitors and analyzes unit specific performance improvement data; including annual agency review, complaints, incidents, and develops Quality Improvement and Performance Improvement Plan.
  • Provides input and assures compliance with agency policies and procedures, scope of care, performance improvement, safety, environmental, and infection control standards.
  • Promotes and maintains an environment that is in compliance with state, federal and regulatory requirements.
  • Provides direct support and administrative control of any agency branches or alternative site locations on a daily basis.
  • Responsibilities include interviewing, hiring, developing, training, and retaining employees; planning, assigning, and leading work; appraising performance; rewarding and coaching employees; addressing complaints and resolving problems.

Required Education, License/Certification, or Work Experience:

  • Social Worker- Licensing Board BSW or MSW. An Active license in the state of practice Upon Hire or
  • Physical Therapist- Licensing Board An Active license in the state of practice Upon Hire or
  • Registered Nurse (RN) - Board of Nursing An Active license in the state of practice Upon Hire or
  • State Activity Coordinator Certificate- Certification Board An Active license in the state of practice Upon Hire or
  • Nursing Home Administrator- Licensing Board An Active license in the state of practice Upon Hire
  • 1-3 years supervisory or administrative experience in home health care or related health programs
