Job Description

OBJECTIVE: Provides Activities of daily Living (ADL) and Instrumental Activities of Daily Living (IADL) services to residents in accordance with the Individual Service Plan (ISP). Assistance includes either physical assistance or standby supervision. Provides support to new resident assistants. May provide some laundry and housekeeping tasks. May act as shift leader as established by community standards.

QUALIFICATIONS: The requirements listed below are representative of the knowledge, skill and/or ability required. Minimum Qualifications: Must demonstrate competency in the following areas:

  • High school diploma or general education degree (GED), or three (3) months related experience and/or training or equivalent combination of education and caregiver experience desired.
  • Current Arizona State Caregivers and Med Tech Certification 
  • CPR and First Aid certification or successfully achieved within 30 days of employment if required by State Standards.

    ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: To perform this job successfully, an individual must be able to perform each key function satisfactorily.

    1. Assist resident with all personal care skills (bathing, dressing, grooming, toileting) and services in accordance with Individual Resident Service Plan, using safe, courteous and effective standards of practice.
    2. Makes appropriate observations about resident’s condition in event interaction with resident and records changes in resident’s condition in a timely manner or reports changes to the H &W Director, Nurse or designated charge person immediately.
    3. Records omission of resident services/medications according to company policy/procedures and standards.
    4. Assists in recording temperature, blood pressure, pulse and respiration rates, and food and fluid intake and output, as directed.
    5. Promptly respond to resident call lights.

    6. Demonstrate a courteous, polite, and friendly attitude with families, visitors and co-workers.

    7.  Administers and Manages medication under the supervision and training of the Health and Wellness Director.

    8.  Demonstrates ability to read, write, and speak English sufficient to fulfill job requirements.

    9.  Demonstrates the ability to make independent decisions, solve practical problems that may or may not include a variety of concrete variables. This may require the ability to interpret instructions given in written, oral, diagram, or scheduled form.

    10. Demonstrates the ability to listen actively and evaluate social situations in an objective, consistent and fair manner. Functions independently with integrity, flexibility and works well with residents, personnel and support agencies.

    11. Complies with community policies and procedures for workplace safety including infection control procedures, emergency response and safety protocol.

    12. Complies with all company policies and procedures related to resident rights; abuse prevention and reporting; and resident confidentiality (HIPPA).

    13. Demonstrates knowledge of Employee Handbook and complies with the policies in the Handbook. 16. Performs other related duties as may be assigned.

    Responsibilities

    • Provide great resident care and support with daily activities and personal hygiene
    • Answer patient calls and determine how to best help them
    • Ensure residents receive appropriate diet by reviewing their dietary restrictions, food allergies and preferences
