Job Title: Apartment Services Caregiver

Overview

The Apartment Services Caregiver provides essential, compassionate, and personalized care to residents within an apartment setting. This role supports the daily physical, emotional, and social well-being of residents, ensuring they maintain the highest possible quality of life in a safe and supportive environment.

Key Responsibilities:

Health Monitoring and Documentation

• Observation: Monitoring and reporting any changes in a resident's physical, mental, or emotional condition to the Apartment Services Coordinator..
• Medication Management: Providing timely and thorough medication monitoring support.
• Doctor Appointments: Attending scheduled appointments and providing Team Lead with updates.
• Documentation: Completing accurate and timely records of all care provided, observations made, and incident reports using the designated electronic or paper system.

Apartment and Personal Care Support

The Caregiver is responsible for providing guidance, support, encouragement and prompting.

• Tidiness: Ensuring the resident's apartment is safe, tidy, and organized.
• Laundry: Ensuring the resident’s laundry is maintained.
• Nutrition: Ensuring the resident has access to nutritious food and is eating regular meals.  Helping with meal planning and grocery shopping.
• Bathing and grooming: Ensuring the resident completes activities of daily living.

Social and Emotional Support

• Engagement: Encouraging and assisting residents to participate in scheduled social activities and programs.
• De-escalation: Providing active listening to resident concerns, and addressing day to day needs (i.e. minor roommate concerns and shopping requests).
• Transportation: Assisting with transportation as needed including, grocery shopping, doctor appointments, work transportation and bus training.

Communication

• Attend and participate in quarterly meetings and as requested.
• Regular communication and collaboration with other caregivers, Apartment Services Coordinator and Apartment Services Manager.
• Comply with all Electronic Visit Verification (EVV) steps.
• Collaborate with members of the client's care team, under the guidance of Apartment Services Coordinators.
• Reading and completing house logs, resident documentation, incident reports and schedules.
• Carry and answer a company cell phone during shift.
• Report any issues or concerns immediately to the Apartment Services Coordinator.
• Consult the on call supervision schedule and contact the designated individual for the shift.

Other Duties 

• Complete all scheduled daily tasks, seeking clarification from Apartment Services Coordinators as needed.
• Ensure all company vehicles are maintained, clean and fueled.
• Other duties as assigned by Apartment Services Coordinator or Apartment Services Manager.

Qualifications

• Must have and maintain a valid driver’s license in good standing and be able to be an approved driver under the company’s liability insurance coverage.
• Must have a minimum of a high school diploma or equivalent.
• Must be able to give and receive feedback in a professional manner.
• Must be proficient in reading, writing and speaking English.

Working Conditions and Physical Demands

• Must be able to lift at least 25lbs.
• Must be able to sit for extended periods in an office environment or in a vehicle.
• Must be able to traverse in home, office and community settings.
