Job Description

Concord Hospital Medical Group is seeking a compassionate and collaborative experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Rheumatology team. This full-time position offers a balanced opportunity to practice in both Concord and Laconia, caring for patients across the Lakes Region and Capital Area of New Hampshire.

This role is ideal for an experienced APP who values patient education, team-based care, and developing long-term relationships with patients managing chronic rheumatologic conditions.

About the Role

As part of our Rheumatology practice, you will care for patients with a wide range of autoimmune and inflammatory conditions, including but not limited to rheumatoid arthritis, lupus, psoriatic arthritis, gout, and vasculitis. You'll work closely with experienced rheumatologists and a multidisciplinary care team to deliver high-quality, coordinated care.

Key Responsibilities

  • Perform comprehensive patient evaluations, including history, physical exams, and diagnostic assessments

  • Manage and monitor patients with chronic rheumatologic conditions

  • Develop and adjust treatment plans, including medication management and monitoring

  • Review and interpret laboratory and imaging results

  • Provide patient and family education on disease processes, medications, and lifestyle considerations

  • Collaborate with physicians and other care team members to ensure seamless care

  • Participate in quality improvement initiatives and clinical program development

Qualifications

  • Licensed Nurse Practitioner or Physician Assistant in New Hampshire (or eligible for licensure)

  • Prior experience in rheumatology and internal medicine/primary care strongly preferred

  • Strong clinical judgment, communication, and organizational skills

  • Commitment to patient-centered care and education
