Position: Assistant Director of Nursing (ADON)/ MDS Coordinator
Rochester West, a skilled nursing facility located in Rochester MN, is searching for an Assistant Director of Nursing (ADON)/ MDS Coordinator.
The Assistant Director of Nursing (ADON) / MDS Coordinator plays a critical role in supporting the day‑to‑day operations of the nursing department to ensure high‑quality resident care in compliance with federal, state, and local regulations. This position provides ongoing leadership support through staff development, clinical oversight, regulatory compliance, and interdisciplinary collaboration.
The role includes on‑call responsibilities and may require occasional floor coverage as needed.
Responsibilities:
- Assist in developing and implementing nursing policies, procedures, and regulatory compliance initiatives.
- Support staff development, training, onboarding, and performance improvement.
- Oversee infection control practices and wound care management.
- Conduct routine nursing rounds to monitor care quality and support staff.
- Ensure resident rights, safety, and protection from abuse or neglect.
- Participate in interdisciplinary team meetings and assist with operational planning.
- Collaborate with the DON to mentor nursing staff and maintain a positive team culture.
- MDS scheduling, set up and completion
- New admission authorizations, updates, communication with family/insurance providers, communication with case managers
- ICD 10 coding for all new admissions and continual updating for LTC population
- Participate in the implementation, updating and review of the resident care plan
- Attend/communicate with family’s and residents on current plan of care, insurance needs, and discharge needs – attend care conferences
- Educate IDT on needs of the MDS, Assessments and Skilled care service requirements
- Run Utilization Review Meetings and ensure PDPM scores and rates are validated
- Review and Educate IDT/Staff to ensure proper documentation is maintained
- Maintain Insight/Participation to facility Quality Measures and QAPI process
- Chart Reviews/Audits as needed
- Assist IDT during Survey process
Education and/or Experience:
- Active LPN or RN license in the state of Minnesota
- 2+ years of experience in a Skilled Nursing Facility setting
- MDS experience preferred
- Prior nursing supervisory or leadership experience preferred
- Experience with applicable state Survey Rules Regulations, Medicare and Medicaid.
- Ability to use desktop/notebook computers, Microsoft Office products, and various medical equipment required.
- Prior experience with PCC (Point Click Care) a PLUS!
