Job Summary:
Program management responsibilities include serving as the Behavioral Health Program Coordinator. Coordinates with leadership to ensure quality and regulatory compliance functions as they relate to the care of behavioral health patients and to the behavioral health holding area. This includes assistance with policy development which meets regulatory requirements. Responsibilities include participating in interviewing and making recommendations related to hiring and reporting directly to leadership regarding care concerns or non-compliance with policy and processes. In addition, functioning as sitter for mental health patients when needed and offering inpatient mental health resources.
Quality Improvement responsibilities include ensuring that behavioral health services are in compliance with regulatory requirements, policy development, auditing files, addressing consumer complaints, informing and training staff on new policies and procedures.
Provides a wide range of care coordination functions to include referrals and discharge planning under the guidance of the provider. Provides referral for consumers seeking mental health or social services in the emergency department, inpatient and 2East. Arranges consults and provides education and information to families, providers, and community stakeholders regarding individual situations and treatment or community resources. Functions as a sitter for mental health patients when necessary.
Advocates for patients. Connects patients to appropriate resources as developed by treatment plan. Assists with coordinating patient care, e.g. transportation arrangements, medical appointments, referrals, preventive care, education and/or follow-up care for the emergency department, 2east and inpatient units.
Works with multidisciplinary team (RN, MD, APP, LCSW, Psychiatrist, Psychologist, LCT) to support patients and to address the social determinants that impact the patients' health by linking the patient with resources (housing, transportation, healthcare). Works with patients facing homelessness, substance abuse, mental health issues, and or complex chronic medical conditions.
The Behavioral Health Coordinator will serve as a resource within the Emergency Department, assisting with EKG team troubleshooting, conducting chart audits, and supporting additional operational and clinical workflow processes as needed.
Education:
Required:
- High School Diploma
Preferred:
- Bachelor's Degree in Psychology or Social Work
- Bachelor's Degree in a Human Service field
Experience:
Required:
- Working with adults and children
- Working with community and NC State resources
- Working within team
- Working with the public
- Working within the health care or mental health field
Preferred:
- Working in an emergency department setting
- Working in a mental health setting
- Working in a social service setting
Position Requirement(s): License/Certification/Registration
Required:
- BLS completed prior to going to department/unit and maintained going forward
- Crisis intervention and de-escalation training within 30 days of hire and maintained thereafter as required by organization

Department Position Summary:

Job Duties:
Serves as the Behavioral Health Program and Coordinator
Coordinates with leadership to ensure quality and regulatory compliance functions as they relate to the care of behavioral health patients and the behavioral health holding area to include assistance with policy development which meets regulatory requirements
Gathers information to include patients' social, psychological, cultural, environmental and financial situation; identifies legal, financial, social, educational and environmental factors which may affect medical care and/or discharge plans
Communicates to care team critical information that may affect the patients' stay while in the hospital and/or the discharge plan
Ensures an appropriate environment and develops and provides activities for behavioral health patients; encourages daily participation of group or individual activities as appropriate
Provides care coordination with MCO's, Public Health Department, providers, and community stakeholders regarding crisis stabilization and intervention as well as appropriate resources for referral
Assures appropriate reporting of any potential/actual abusive relationship, such as child/adult abuse, neglect or domestic violence
Coordinates telepsych and teletherapy consults of Emergency Department patients on an ongoing basis
Coordinates in-patient psychiatric consults as requested by provider
Upon request of provider or patient, initiates or assists in arranging appropriate intervention and referrals for treatment upon discharge
Provides monthly stats to NCSTeP ongoing
Assists Physician to petition for involuntary commitment when clinically indicated
Facilitates patients' access to services that will increase better health outcomes by provision of client and family education, sharing information with other agencies/providers and/or linkage to resources and services
Facilitates coordination/development of appropriate safety plan/measures if patient's plan is to be discharged to the community
Communicates documentation and treatment plan with the referring provider on an ongoing basis to assure continuity of care
Provides support for difficult discharge placements of behavioral health inpatients
Receives referrals for placement of ED patients to post-acute care facilities and interviews families to assist with appropriate placement; facilitates referrals as appropriate
Ensures appropriate discharge documentation is available to accompany patient to the receiving facility and coordinates appropriate transportation
Documents, accurately and thoroughly
Participates in hospital based committees as requested by leadership
Represents EWCH behavioral health services by participating in community/NC State-based committees
Works with Business Development and Marketing to develop an outreach plan to provide education to partners and community members regarding the program
Performs other duties as assigned that the employee is capable of performing
