Join Our Team at Valley-Wide Health Systems, Inc.
Valley-Wide Health Systems is seeking a compassionate and collaborative Behavioral Health Provider to deliver integrated, whole-person behavioral health care within our team-based primary care model, supporting patients' mental, emotional, and social well-being across the lifespan.
Key Responsibilities
- Serve as a Behavioral Health Provider (BHP) within a team-based care (TBC) model, delivering whole-person (biopsychosocial) care.
- Identify and assess at-risk patients, including crisis evaluation and intervention.
- Conduct diagnostic mental health assessments and screenings; determine level of care and refer to specialty services as needed.
- Develop and implement biopsychosocial treatment and relapse-prevention plans in collaboration with patients, families, and care teams.
- Provide evidence-based behavioral health interventions and psychotherapy.
- Monitor patient progress, ensure timely and accurate documentation, and communicate updates with the interdisciplinary care team.
- Collaborate with internal and external partners, including Enabling Services, to address social determinants of health.
- Participate in integrated care meetings and support the ongoing development of integrated behavioral health infrastructure.
- Maintain professional relationships with community agencies, healthcare partners, and local organizations.
Requirements
- Master's or Doctoral degree in psychology, counseling, or clinical social work.
- Licensed or license-eligible in Colorado as a Behavioral Health Provider (LCSW, LPC, LMFT, PsyD, or PhD).
- LPCCs must complete the Jurisprudence Exam and obtain LPC licensure within three years of hire.
- Two years of clinical experience preferred.
- Knowledge of mental health assessment, psychotherapy, crisis intervention, and DSM-5/ICD-10.
- Ability to pass a behavioral health background check.
- Valid Colorado driver's license with ability to travel.
- BLS certification provided upon hire.
