Community Bridges is seeking a Full-Time Behavioral Home Direct Support Professional (DSP) to join our team in Northfield, NH.
Founded in 1982, Community Bridges is a nonprofit organization dedicated to advancing the integration, growth, and interdependence of people with developmental disabilities within their own communities. We support more than 1,100 individuals and families throughout the Greater Capital Area by identifying barriers to access and developing creative, meaningful community connections.
Position Overview
The Behavioral Home Direct Support Professional serves as a community connector, supporting individuals through community mapping and engagement to promote social inclusion. This role focuses on identifying each individual's strengths, interests, and gifts to foster meaningful relationships and community involvement-promoting abilities, not disabilities.
Qualifications
Education: High School Diploma or GED required
Experience: Experience in human services or related fields supporting individuals with disabilities preferred
Reliable transportation, valid driver's license, and proof of automobile insurance required
Compensation & Benefits
$1,000 Sign-On Bonus
$500 paid at hire
$500 paid after 6 months of employmentSign-on bonus available to new hires only. Re-hires and transfers are not eligible.
Paid Time Off:
22 days of PTO per year for full-time employees

Current Openings
Full-Time: Monday–Friday, 3:00 p.m.–11:00 p.m. (1 position)
Full-Time: Monday–Sunday, 11:00 p.m.–7:00 a.m. (2 positions)
Part-Time: Saturday & Sunday, all shifts (1 position)
- Part- Time: Saturday, 7am- 3pm, & 3pm- 11pm (1 position)
Part-Time: Monday & Thursday, 7:00 a.m.–3:00 p.m. (1 position)
Part-time shifts may be combined to create full-time hours.
Key Responsibilities
Support individuals with planned activities and community involvement
Develop relationship maps to identify social inclusion opportunities
Provide supervision and intervention to ensure safety in the community
Assist individuals in building and maintaining meaningful relationships
Promote self-esteem through active community participation
Provide individualized supports including personal care and skill acquisition
Assist with hygiene, bathing, toileting, and personal care as needed
Participate in person-centered planning and goal implementation
Complete required documentation, data collection, and progress notes
Assist with training and orientation of new staff
Administer medications in accordance with He-M 1201, as applicable
Report safety, health, or human rights concerns immediately
Ensure compliance with state regulations
Perform other duties as assigned
Required Skills

Strong verbal and written communication skills
Ability to work independently and exercise sound judgment
Ability to meet documentation deadlines
Ability to support daily living and functional skill development
Ability to utilize community resources and social opportunities
Ability to work collaboratively as part of a team
Basic computer skills, including email and word processing
Professional, positive representation of Community Bridges
Work Environment & Physical Requirements
Primarily indoor environment with occasional outdoor activities
Periods of increased noise and stressful situations may occur
Must be able to lift up to 50 lbs.
Frequent walking, standing, stair climbing, and transfers
Ability to push wheelchairs and operate lifts (Hoyer/barrier-free)
Emergency behavioral interventions required per individual plans
Travel Requirements
Access to a personal vehicle during scheduled work hours
Frequent travel with supported individuals using personal or agency vehicles
Valid NH driver's license and acceptable driving record required
