Overview:

Sentara Service Line Administration is now hiring a full-time Cardiac Care Continuum Navigator in Virginia Beach, VA!

Hours

Monday-Friday 8:00AM - 5:00PM

This is a hybrid position, offering the flexibility to work both remotely and in person. This position requires three days per week on site and two days remotely (at the candidate's discretion).

Overview

The SR. Navigator is responsible for maintaining and reviewing quality metrics and fallouts, program development, participating in system initiatives, data analysis, maintaining programs and performing duties appropriate to the navigator's assigned service line. The SR. Navigator on-boards new team members and provides guidance and mentorship to other navigators.

Daily activities include those duties required to ensure the ongoing operational and managerial elements of the program are running smoothly. Daily activities typically account for 60% of the SR. Navigators time and include: daily rounds, discharge planning, physician relations, pre-operative chart/pre-admission testing process and monitoring the destination center surgery schedule. Program development activities include the activities necessary to ensure the long-term success and growth of the program. Program development activities typically account for 40% of the Navigators time and include: patient education, program development, patient satisfaction, continuous improvement and data collection and marketing.

Education

• Bachelors Level Degree (Required)- Nursing, Healthcare Administration, Public Health, Business Administration, Clinical Healthcare, or related field.
• 5 - 10 years of relevant work experience in lieu of education, however candidate must obtain BS degree within 18 months of hire.
• For Registered Nurse candidates, BSN (Required).

Certification(s)

Must hold one of the following certifications:

• Certified Occupational Therapy Assistant (COTA) - Certifications- National Board Certification In Occupational Therapy (NBCOT)
• Licensed Nurse Practitioner (NP) - Certification- Other/National
• Licensed Nurse Practitioner (NP)- Nursing License- Virginia Department of Health Professionals (VADHP)
• Occupational Therapist License (OT) - State license- Virginia Department of Health Professionals (VADHP)
• Physical Therapist Assistant License (PTA) - State license- Virginia Department of Health Professionals (VADHP)
• Physical Therapist License (PT) - State license- Virginia Department of Health Professionals (VADHP)
• Physician Assistant License (PA) - State License- Other/National
• Physician Assistant License (PA) - State License- Virginia Department of Health Professionals (VADHP)
• Registered Nurse (RN) Multi State- Nursing License- Virginia Department of Health Professionals (VADHP)
• Registered Nurse (RN) Single State- Nursing License- Virginia Department of Health Professionals (VADHP)

Experience

• Clinical- 3 years (Required)
• Ortho Neuro Navigator Experience- 2 years (Required)
• Orthopedics, Neurosciences or Musculoskeletal Experience (Preferred)

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $105,310.40 - $150,633.60 annually.

Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
