Job Summary

The Baltimore City Health Department (BCHD) seeks a Grant Service Specialist II Cardiovascular Disease Program Coordinator to work with the Office of Chronic Disease Prevention in the Bureau of Health Promotion and Education. Founded in 1793, BCHD is the oldest health department in the country, with over 800 employees and an annual budget of $208 million, that aims to protect health, eliminate disparities, and ensure the well-being of every resident of Baltimore through education, advocacy, and direct service delivery. BCHD's wide-ranging responsibilities include maternal and child health, youth wellness, school health, senior services, animal control, restaurant inspections, violence prevention, emergency preparedness, STI/HIV treatment, and acute and chronic disease prevention.

Essential Functions

The Baltimore City Health Department is seeking a Grant Service Specialist II to act as the Cardiovascular Disease Program Coordinator in the Office of Chronic Disease Prevention. The GSS II will collaboratively develop a standardized process to identify individuals at risk for hypertension, screen them for social support needs (with a specific focus on food security) and provide navigational support and assistance. This GSS II position is a Community Health Worker (CHW) position and will connect participants currently enrolled in the Healthy Heart Ambassador Program and other lifestyle change programs to services that address social determinants of health, which may not be met in their current program. The GSS II will provide education, encouragement, and support to participants who engage in any of the above programming.

The selected candidate will be supervised by the director of the Office of Chronic Disease Prevention. Specific duties and activities include:

  • Establishing and maintaining relationships with existing programs/organizations
  • Implementing cross-referrals with those and the Healthy Heart Ambassador Program.
  • Providing hypertension assessments to people at high risk for hypertension at partnering programs or organizations
  • Recruiting participants to the Healthy Heart Ambassador Program
  • Facilitate monthly nutrition seminars in the Healthy Heart Ambassador Program, if needed
  • Maintaining and drafting necessary documentation and program records and reports
  • Establishing new Healthy Heart Ambassador sites
  • Referring participants to programming and following up with those participants to ensure social needs are met

Minimum Qualifications

Education: Have a degree from an accredited college or university.

AND

Experience: Have two years of experience giving presentations and facilitating discussions is required. Experience with motivational interviewing is preferred.

Have experience working in the community/as a Community Health Worker is preferred.

OR

Equivalency Notes: Have an equivalent combination of education and experience.

Licenses, Registrations, and Certificates: Have a valid Maryland Class C Noncommercial driver's license or an equivalent driver's license and be eligible to obtain a Baltimore City driver's permit. PROVISIONAL DRIVER'S LICENSES ARE NOT ACCEPTABLE.
