Job Description

PACE, a member of Empath Health is seeking a compassionate and detail-oriented Care Coordinator to join our interdisciplinary PACE team in Pinellas Park, FL. In this role, the PACE Care Coordinator is responsible for service coordination and communication to support participants throughout the care continuum. This role organizes appointments, services, and information flow between participants, caregivers, providers, and the care team.

  • Team-Driven, Preventive Focus: Collaborate closely with physicians, social workers, therapists, and other clinicians in a proactive care model focused on prevention, wellness, and holistic well-being.

What You'll Do

  • Serve as the central communication point for participants, caregivers, providers, and team members.
  • Support transitions of care, including hospital discharges, SNF stays, ER visits, and facility moves.
  • Arrange and track home care visits; follow up on missed or incomplete services.
  • Communicate with long-term care facilities to obtain records and route documentation.
  • Assist with disenrollment tasks such as medication supply, service discontinuation, and DME pickup
  • Confirm specialist and procedure appointments, share instructions, arrange transportation, and document completion.
  • Maintain timely, accurate documentation in the electronic record system.
  • Ensure compliance with CMS, ADC, HIPAA, and organizational standards.

What You'll Need

  • High school diploma or equivalent required.
  • Two (2) years of experience in healthcare, service coordination, and customer service required.
  • Strong communication skills, customer service mindset, and organizational abilities.
  • Ability to manage multiple tasks in a fast‑paced environment.
  • Must be able to communicate effectively by phone.
  • Proficient computer skills and ability to work in electronic record system.
