This position ensures the education of the care management team while maintaining current skills sets in assisting Complex Care Managers. The Care Management Education Specialist on-boards and assists in orienting new colleagues joining the department. The Care Management Education Specialists ensure new colleagues understand the policies and procedures of the department and serves as a mentor for one year following the colleagues start date. This position maintains current skills sets by assisting/maintaining an assignment with the Complex Care Managers.Job Description
This role requires frequent travel to different sites across the region.
ESSENTIAL FUNCTIONS:
- Ensures new processes are reviewed and understood by all colleagues and evaluates current processes.
- Serves as primary oversight in precepting new care management colleagues.
- Assists the Enterprise Director of Quality and Education in arranging and leading education opportunities.
- Prepares on-boarding schedules for new colleagues.
- Meets with both colleague, director/manager and floor preceptor at a regular cadence to review orientation process.
- Continues as primary mentor for one year for new colleagues.
- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson
- Assists in the review, development and implementation in department procedures, standard of work and regulatory
- process.
- Analyzes and interprets quality reports and data collection related to staff compliance and workflow optimization.
- Maintains small case load of complex care patients.
Education- Required
- Bachelor’s Degree in Nursing or Graduate from an accredited Masters of Social Work program required.
Experience:
- 1 year precepting care management colleagues and
- 3 years inpatient care management experience.
Knowledge, Skills and Abilities:
- Precepting colleagues.
- Public speaking and presenting.
- Problem solving and critical thinking skills.
- Knowledge, Skills and Abilities- Preferred
- Experience in Powerpoint and Excel.
- Knowledge of the Conditions of Participation for Discharge Planning.
- Knowledge of Medicare required documentation.
Licenses and Certifications Required:
- RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire
- Licenses and Certifications- Preferred
- ACM- Accredited Case Manager- American Case Management Association within 3 Years or CCM- Certified Case Manager- Commission for Case Manager Certification within 3 Years
